CareVector™ Portal

CareVector has been greatly enhanced to make it better and easier for you.  At first, it may seem a bit confusing but you will see the benefit and potential right away.  We will help orient you to the new components and features.
 
CareVector will now have three parts – each part uses more readily available tools that are easier to develop and maintain.  For A and B you can enter and use your email address and any password – they are separate components each require their own log in.  To send us documents securely, no log in is needed.
 
    1. Messaging: CareVector Portal is for you to send/receive messages securely and for you to receive files. This messaging will replace email because, unlike email, it is fully secured and private. Username and Password information will be given to you securely in a phone call.

       

    2. Status Update Forms and Medication Updates:  This will be a dynamic form that will capture your status and prompt you with a much simpler interface to let us know about your key medications and supplements. Again, please use your email address as the username and you are welcome to use any password you want.

       

    3. Sending us documents securely – can now be done without logging in – just click and send any document and we will be notified.  No username or password is needed for this.
You will have access to the old CareVector portal until December 15, 2020 after which time the old files will be available to you only on special request.

To get the most out of your appointment, being prepared is key.

Please do your preparation for your upcoming appointment

Please complete the following:
1. Re-read the last dictated note from Dr. Mozayeni or Beth Renne, NP
2. Submit, AT LEAST 1 week in advance, your status update (Click Here).  This written update saves the practitioner time and saves you money.  These details are very helpful to the practitioners before each appt.  This form is a fillable pdf • It is NOT acceptable to just state nothing has changed! Please have someone assist you with this if you need it.
3. Use this template and submit an UPDATED complete medication and supplement list (Click Here) in the new required format for all medications/supplements you are currently taking listed by medication name, dose and then how many times daily you take it. This form is a fillable pdf • It is NOT acceptable to just state nothing has changed! Please have someone assist you with this if you need it. It is a crucial aspect of your treatment to know exactly what you are taking.
